Electronic Reserve Request Form

Use this web form to submit a request to place course materials on electronic reserve via OAK.
Note: For the library to post items to your course, you must allow "course builder" access to your course.
OAK (Blackboard) - materials are usually scanned/linked in the E-Reserves content area of the course.

Whenever possible the library will attempt to link to the requested material in a licensed electronic resource rather than scan the document, thus
avoiding copyright clearance costs and the time that is required to obtain copyright permissions from the rightsholder.

Electronic Reserves documents are usually scanned and stored on OAK as Adobe PDF (portable document format) files. Access to these documents is through a web browser. Viewing/printing of the scanned document requires that the Adobe Acrobat Reader plugin be installed on your computer. To get the Adobe Acrobat Reader click the image below.
NOTE: Fields with bold labels are required.
Please complete the form below to submit a request to have electronic reserve materials processed on the OAK: Online Access to Knowledge (Blackboard) system. Please deliver clean, unstapled, single-sided paper copies of the materials listed on this form to the library in person for scanning. You will receive a confirmation email from the reserves supervisor in response to your submission, together with notification of what, if any, copyright permissions must be obtained for the materials. When the materials have been processed on OAK you will be notified by email.
Copyright Policy: permissions must be obtained for all copyright-protected electronic reserve materials posted to OAK. The VU Copyright Clearance Services unit in the General Library Building will request copyright permissions for electronic reserve materials. OAK courses are password protected by VUnetID and e-password and are limited to the students enrolled in the course. Please visit the Copyright Clearance Services web site for further information.
